Title: New version of my DS order form

I suddenly realised that the DS order from I posted in this thread:

wasn't the best version for the job.  Originally, I'd devised the form for my own use and then adapted it for use by people who were ordering DS items through me.  So here's a revised version for people to use for their own orders with their own DS service.  The main body of the form is the same and it's just the detail box at the top and the footer which have been changed.
