It is the head/hat/hair-piece of the eyemasked "superhero" Surprise Figure from Series 5 (Boys).
There was also a swim-figure in the Olympics' series which came with this headpiece as a swimcap, but then it had another colour I believe.
The head itself is the Figures-5 Ninja's one, with the "masked" part painted black, and with an additional cowboy's scarf over the mouth.


In case anyone wonders about the black arms with black hands: those come from the (alienlike) statue-figure in the Treasure Hunters' Temple 4842.
